School Nurse

Nurse Erin Holbrook, RN BSN

My name is Erin Holbrook, and I am the school nurse at Silver Creek Elementary School. 

My hours are Monday- Friday, 7 am- 2:30 pm. You may contact me anytime within those hours for health-related questions or medical concerns regarding your student here at SCE.

Please remember to make sure all health requirements for school are met. It is essential in helping your child be ready for school. It is the school nurse's responsibility to monitor compliance for well-child physicals, immunizations, sports physicals, and screenings (vision/hearing).

It is a requirement by Kentucky for all students to have an up-to-date, valid immunization certificate on file at school at all times. 

I love being part of the Bobcat family and enjoy taking care of our Bobcats each day!

Consent Forms:

If your child has prescription medications or over the counter medications that will need to be taken at school, a signed Medication Consent form is required.

• Liquid Tylenol or Motrin (Generic is fine)

• soft mints

• 3 oz drinking Dixie cups (Generic is fine)
